I understand that from time to time, candid, posed or staged photographs and/or videos of students are taken at different events/classes. All enrollees are asked to release Unity Worldwide Spiritual Institute and Unity Worldwide Ministries from any and all claims so that these photos and/or videos may be used, at the discretion of Unity Worldwide Spiritual Institute and Unity Worldwide Ministries, for promotional materials such as but not limited to, publications, brochures, press releases, Unity Worldwide Spiritual Institute’s webpage, and Facebook. My signature below releases Unity Worldwide Spiritual Institute and Unity Worldwide Ministries from any and claims arising from use of photos and/or videos.
By signing below I certify that the information provided in this application and any attached documentation is true and accurate to the best of my knowledge. I understand that any omissions or falsifications may result in the withdrawal of a decision to accept me. I also understand that my application materials, once submitted, become the property of Unity Worldwide Spiritual Institute and will not be returned or photocopied. My signature below acknowledges that I waive my right to inspect any reference letters submitted on behalf of my application.
